Is your
child going into 6th Grade?
Effective
September 1, 2007, students who are 11 years old and entering
6th grade must receive a booster vaccine containing
tetanus toxoids, diphtheria and acellular pertussis (Tdap).
Ten year old students entering 6th grade must have this vaccine
when they turn 11-years old.
In
addition to the Tdap vaccine, students entering 6th grade in the
fall must provide proof of immunization against Varicella
(chickenpox) or provide documentation from their physician
stating that they have had the chickenpox disease.
