Reminder
Physicals
Any student entering grades two, four, seven and ten in September
must have a physical examination according to New York State
law.
If
you would like your child examined by his or her doctor over the
summer, please have your doctor complete
this form after your child
has been examined. The form must be returned to the
school nurse by October 1 or the school physician
will examine the student.
All Kindergarten students and new entrants to Schenectady City
School must be examined by their own physician and
this form returned to the
school nurse by the first day of school. Physicals
completed within 12 months prior to the beginning of the school
year for which the exam is required, will be accepted.
